Registration Process
To register for Workshops or the Certificate in Food Service Management classes:

  1. Complete one registration form for each individual registering.
          Note: You will need Acrobat Reader in order to download the registration form.
            If you do not have Acrobat Reader you can
    download a free version from Adobe.

  2. Make checks payable to The John C. Stalker Institute. Note: A check or Purchase Order must accompany each registration form.
  3. Mail check or Purchase Order and registration form to:

Confirmation of registration and directions will be mailed or faxed at least one week before the workshop or class begins.

Cancellation Policy: A voucher for another John C. Stalker Institute workshop will be issued if written cancellation notice is received in writing two weeks prior to date of workshop. No cash refunds will be made. If a workshop is cancelled and not rescheduled by the John C. Stalker Institute, a full refund will be given.
